Physician Face-to-Face Encounter


The Affordable Care Act requires physicians certify eligibility before patients can be referred for services at SSM Health at Home.

That face-to-face encounter requirement must occur within the 90 days prior to the start of care, or within the 30 days after the start of care. If a physician works in collaboration with a nurse practitioner, clinical nurse specialist or supervises a physician's assistant, the encounter may be undertaken by them. However, only a physician can order home health care services.

Certification must include the following information:

  • The date of the encounter.
  • Indication that the encounter was related to the primary reason for home health services.
  • Clnical findings support the need for skilled nursing and/or therapy.
  • Clinical findings that that the patient is homebound.
  • Your signature and date of signature.
